Intake Assessment

An intake assessment is necessary when starting a therapeutic process. We will cover topics ranging from client rights to the reasons you are seeking counseling, gather relevant information and begin the collaborative process of treatment planning.

Individual Therapy

Individual therapy sessions will unfold based on a combination of factors that include your individual needs and wants, evidence-based practices based on presenting issues, and the transparent and mutually accepted treatment plan. FourEight Transitions will always strive to be your partner on the path to wellness.

Fees

Fees are in accordance with the practice costs for service. Payment is due at the time of service. Accepting Aetna, Moda, Providence, and Regence BC/BS commercial insurance plans. Medicaid and Medicare plans are accepted through Medicaid Open Card, AllCare Health, CareOregon, Jackson Care Connect, and HealthShare.

• Individual Therapy (50 minutes): $175-$275
